Logic behind the LyX LaTeX font menu

When trying to the make STIX Font appear in the LyX font menu I had cause to look at the default LaTeX fonts and what packages they were sourced from

Does anyone know why these fonts were selected?  What it with TeX Gyre?
Can a style expert recommended which to use together for good style (ie: which rm, tt , sf, math go well together)?
Some font did not seem to call the package by name. How do they link?
Some like Zapf Chancery are not in MikTeX Distribution, so why, and how, are they included?

I want to get the most out of LaTeX to typeset the best documents. There are probably mistakes in the list and comments below, so please take is as a draft and state errors if you find any.
